That brings the active code set to 79,258 codes. Facilities still running June files are coding against an incomplete set.

Why Small ICD-10-PCS Updates Carry Enterprise-Level Revenue Risk

ICD-10-PCS codes feed the MS-DRG Version 44 grouper, which sets inpatient reimbursement for every Medicare discharge. One wrong seventh character can shift a case into a lower-weighted DRG.

Section X codes carry a second exposure in the ICD-10-PCS updates. They are how payers see that a qualifying technology was used, so a missed X code can mean a missed NTAP payment even when the device was implanted. For hospital systems and PE-backed groups, that leakage compounds across every service line over 12 months.

Section X ICD-10-PCS Updates: Where Half the Change Lives

Of the 101 original additions, 50 landed in Section X across 13 new tables, all tagged New Technology Group 12 (seventh character “C”). The errata added XW0U3CC and XW0U4CC, which cover introduction of recombinant human bone morphogenetic protein-2 with collagen scaffold into joints. Both are non-O.R. codes that do not change DRG assignment.

All 38 deletions came from New Technology Groups 7 and 8 (FY 2022 and FY 2023). CMS retired some technologies outright and moved others into the main sections. Coders who memorized those X codes are now reporting invalid values.

XEZ Table: AI and Software Codes in the ICD-10-PCS Updates

The new XEZ table classifies software-as-a-medical-device technologies, such as AI detection of echocardiogram, ECG and vital-sign abnormalities. These codes only apply when the record names the software, which makes XEZ a clinical documentation improvement problem before it becomes a coding one.

The ICD-10-PCS Official Guidelines saw no significant changes. The parallel ICD-10-CM update adds 190 new billable codes, 4 revisions and 30 deletions, which is where outpatient and post-acute specialties feel the cycle most.

How the FY 2027 ICD-10-PCS Updates Hit Each Specialty

ICD-10-PCS is inpatient-only, but these ICD-10-PCS updates still reach every setting that sends patients to a hospital or receives them after discharge.

1. Ambulatory Surgery Centers (ASCs)

ASCs bill with CPT and HCPCS, not PCS. The exposure is strategic: new inpatient technology codes signal which procedures may later leave the Inpatient Only list and migrate to outpatient settings.

How MBC helps: Our ASC medical billing services track site-of-service shifts, so payer contracts and case-mix plans are ready before a procedure moves.

2. General Surgery

In the ICD-10-PCS updates, Table 0W9 adds a qualifier for pleural cavity and mediastinum drainage using an active mechanical clearance device. On the diagnosis side, E89.830 and E89.838 now capture hypoglycemia after bariatric and other surgeries.

How MBC helps: Our general surgery billing coders map both changes into charge capture and post-op diagnosis coding, protecting DRG accuracy on surgical admissions.

3. Skilled Nursing Facilities (SNFs)

SNFs bill with ICD-10-CM, but the hospital’s PCS record shows which surgery a resident had. That detail feeds the MDS Section J surgical items that shape the PDPM clinical category.

How MBC helps: Our SNF billing specialists reconcile hospital discharge records against MDS coding, so PDPM classification reflects the actual procedure.

4. Primary Care

Primary care runs on ICD-10-CM, not the ICD-10-PCS updates, but the diagnosis side changed too. New codes include Z86.17 for history of C. difficile and adult BMI codes Z68.18 and Z68.19, and deleted codes become instant rejections if favorites lists are not refreshed.

How MBC helps: Our primary care billing teams update scrubber edits and code favorites, so retired codes never reach the payer.

5. Internal Medicine

Internists managing patients after procedures affected by the ICD-10-PCS updates now have E89.830 and E89.838 for postprocedural hypoglycemia. Transitional care management claims (CPT 99495 and 99496) depend on documentation that reflects the inpatient stay accurately.

How MBC helps: Our internal medicine billing specialists audit TCM documentation against discharge data before claims go out.

6. Family Practice

Family physicians see the diagnosis side of the ICD-10-PCS updates cycle first. The new BMI, C. difficile history and post-surgical hypoglycemia codes affect risk adjustment and quality reporting, so missed specificity understates patient acuity.

How MBC helps: Our family practice billing teams run 12-month diagnosis frequency reports against the deleted and revised lists to close specificity gaps.

7. Dermatology

Inpatient dermatology volume is small, but the ICD-10-PCS updates still raise the stakes. Excisional debridement is coded to the deepest layer reached, so a vague depth note can shift DRG weight even without a new derm-specific table.

How MBC helps: Our dermatology billing coders query depth and layer documentation before inpatient debridement and graft claims are finalized.

8. Plastic Surgery

Reconstructive flaps and grafts often follow trauma or oncology admissions, so the ICD-10-PCS updates matter here. The PCS device character changes with graft source (autologous, synthetic or nonautologous), and that choice affects DRG assignment.

How MBC helps: Our plastic surgery billing teams separate cosmetic from reconstructive work and verify device characters before claims drop.

9. Hospitalist Billing

Hospitalists document most bedside care, so the ICD-10-PCS updates land on their notes first. New XEZ codes such as XEZZXMC (AI-detected ECG abnormalities) and XEZZXRC (cardiac abnormalities from vital signs) only apply when the note names the software used.

How MBC helps: Our hospitalist billing specialists build CDI query sets so physician notes support both pro-fee claims and facility DRGs.

10. Cardiology and Cardiothoracic Surgery

Among the cardiac ICD-10-PCS updates, Table 028 adds aortic and mitral valve body parts with native versus prosthetic leaflet qualifiers. XEZ adds XEZZXLC (AI echo detection), XEZZXPC (3D fluoroscopic navigation) and XEZJ3NC (coronary crossing and re-entry).

How MBC helps: Our cardiology billing coders verify leaflet and technology documentation so valve and NTAP-eligible cases are coded in full.

11. Spine and Orthopedic Surgery

The spine-focused ICD-10-PCS updates in Tables 0RG and 0SG add device values for custom-made interbody fusion devices, anatomically and virtually designed.
