Modifiers explain those circumstances so a claim can be paid when it otherwise would not be. Used incorrectly, though, a modifier triggers denials just as fast as a missing one. Modifier 51 is one of the most frequently misapplied.

This post is part of our series on the 10 most commonly misused modifiers. Below, we explain what Modifier 51 means, when to use it, when not to, and how payers calculate payment for multiple procedures.

Key Takeaways

Modifier 51 reports multiple procedures by the same provider in the same session, on the same day.

Never append it to add-on codes (CPT Appendix D), Modifier 51-exempt codes (Appendix E) or E/M services.

Rank procedures by current fee schedule value: the highest is paid at 100%, additional procedures are typically reduced to 50%.

Medicare and many MACs apply multiple procedure reductions automatically. Check each payer’s policy before appending 51.

Use Modifier 59 or X{EPSU}, not 51, when you need to show a distinct procedural service.

What Is Modifier 51?

CPT defines Modifier 51 as multiple procedures: when the same provider performs more than one procedure (other than E/M services) during the same session on the same day. The primary procedure is reported without the modifier, and Modifier 51 is appended to the additional procedure(s).

Medicare does not require Modifier 51 on most claims. Its claims processing system identifies multiple procedures and applies the payment reduction to the correct line. Many commercial payers, however, still expect the modifier, so payer-specific policy decides whether you report it.

How Payers Reduce Payment for Multiple Procedures

The CMS Medicare Physician Fee Schedule (MPFS) assigns every code a multiple procedure indicator that tells the payer which reduction rule applies:

Indicator 2 (standard surgery rule): the highest-valued procedure is paid at 100% of the fee schedule amount; the second through fifth procedures are paid at 50%. Procedures beyond the fifth are priced by the contractor on a manual review basis.

Indicator 3 (endoscopy rule): endoscopies in the same family are paid using the base endoscopy rule rather than the standard 50% reduction.

Indicator 4 (diagnostic imaging MPPR): the technical component of additional imaging services in the same session is reduced by 50%, and the professional component by 5%.

Indicator 0: no multiple procedure reduction applies.

Always check the current-year MPFS indicator and RVUs before ranking codes, as values change every January.

Common Modifier 51 Mistakes

Appending it to add-on codes. Add-on codes (CPT Appendix D) are already priced as secondary procedures and are exempt from the reduction.

Appending it to Modifier 51-exempt codes. Codes listed in CPT Appendix E must not carry Modifier 51.

Using it with E/M services. Modifier 51 applies to procedures only. Use Modifier 25 for a significant, separately identifiable E/M on the same day as a procedure.

Reporting it on every line. The primary (highest-valued) procedure never carries Modifier 51.

Placing it on the wrong procedure. Appending 51 to the higher-valued code causes the payer to reduce the wrong line, which leads to underpayment.

Using it across different providers. Do not append it when two or more physicians each perform distinct, unrelated procedures on the same patient on the same day.

Confusing it with Modifier 59. Modifier 51 signals multiple procedures for payment purposes. It does not bypass NCCI edits. Use Modifier 59 or the X{EPSU} modifiers for that.

When to Append Modifier 51

The same physician performs more than one surgical procedure at the same session (MPFS indicator 2).

Multiple procedures are performed on the same day but billed on separate claims, and the payer requires the modifier.

The procedure has the lower fee schedule amount of the procedures performed.

The payer’s policy requires the modifier for multiple diagnostic imaging services subject to MPPR.

Modifier 51 Coding Examples

Example 1: Two fracture treatments. A surgeon performs 24500 (closed treatment of humeral shaft fracture; without manipulation) and 23500 (closed treatment of clavicular fracture; without manipulation). 24500 carries the higher fee schedule amount, so it is listed first and 23500 receives Modifier 51 if the payer requires it: 24500, 23500-51. Confirm the current amounts in the CMS Physician Fee Schedule Look-Up Tool.

Example 2: Colonoscopy and EGD in one session. A diagnostic colonoscopy (45378) and a diagnostic esophagogastroduodenoscopy (43235) are performed at the same session. These codes belong to different endoscopy families, so the standard multiple procedure rule applies between them. Report the higher-valued procedure first and append Modifier 51 to the lower-valued one, based on current RVUs.

Example 3: Lesion excision with intermediate repair. A 0.5 cm malignant lesion on the trunk is excised and the wound is closed with a layered closure in the same session. Report 11600 (excision, malignant lesion, trunk, arms or legs; excised diameter 0.5 cm or less) and 12031 (intermediate repair, scalp, axillae, trunk and/or extremities; 2.5 cm or less). Rank the two codes by current fee schedule value and append Modifier 51 to the lower-valued code. Payment depends on carrier policy, and some carriers re-order the lines automatically based on their own fee schedules.

Modifier 51 can be reported alongside other modifiers when appropriate. When it is combined with Modifier 50 (bilateral procedure), follow the payer’s specific policy, as Medicare and commercial plans handle this combination differently.

Modifier errors are one of the most common causes of preventable denials. Modifier 59 (or XE, XS, XP, XU) identifies a distinct procedural service to bypass an NCCI edit when documentation supports it.

7. Can Modifier 51 be used with other modifiers?

Yes, when appropriate.
