{"id":9585,"date":"2019-07-23T14:10:30","date_gmt":"2019-07-23T08:40:30","guid":{"rendered":"https:\/\/www.medicalbillersandcoders.com\/blog\/?p=9585"},"modified":"2020-10-20T11:43:30","modified_gmt":"2020-10-20T06:13:30","slug":"ambulatory-blood-pressure-monitoring-abpm-coverage-expanded-by-cms","status":"publish","type":"post","link":"https:\/\/www.medicalbillersandcoders.com\/blog\/ambulatory-blood-pressure-monitoring-abpm-coverage-expanded-by-cms\/","title":{"rendered":"Ambulatory Blood Pressure Monitoring (ABPM) Coverage Expanded by CMS"},"content":{"rendered":"<p style=\"text-align: justify;\">Centers for Medicare &amp; Medicaid Services (CMS) finalized its national coverage policy for Ambulatory Blood Pressure Monitoring (ABPM) on 02<sup>nd<\/sup> July 2019.\u00a0 ABPM is a non-invasive diagnostic test that uses a device to track blood pressure over 24-hour cycles, allowing a doctor to assess a patient\u2019s blood pressure during routine daily living, instead of when they are sitting nervously on an examination table.\u00a0 ABPM may measure blood pressure more accurately and lead to the diagnosis of high blood pressure in patients who would not otherwise have been identified as having the condition.<\/p>\n<p style=\"text-align: justify;\">CMS received a request from stakeholders to reconsider this national coverage determination.\u00a0 Prior to today, ABPM was covered only for those patients with suspected \u2018white coat hypertension\u2019 under specific conditions.\u00a0 Whitecoat hypertension occurs when a patient\u2019s anxiety from being in a clinical setting causes an increase in blood pressure beyond what occurs outside the clinical setting. Consistent with multi-society professional guidelines, today\u2019s decision expands access to ABPM to include coverage for cases of suspected \u2018masked hypertension,\u2019 which is essentially the opposite of white coat hypertension and occurs when blood pressure measurements in a doctor\u2019s office are lower than measurements taken outside a doctor\u2019s office.<\/p>\n<p style=\"text-align: justify;\">Under its old policy, CMS covered ABPM in patients with suspected white coat hypertension. Blood pressure readings taken from such patients in clinical settings are higher than their real-world figures, creating the risk that they will be treated unnecessarily for hypertension unless ABPM is performed. However, CMS did not cover ABPM in patients with the opposite syndrome, masked hypertension.<\/p>\n<p style=\"text-align: justify;\">Following recommendations from the U.S. Preventive Services Task Force and a formal request from medical associations, CMS has now changed its policy to cover masked hypertension.<\/p>\n<p style=\"text-align: justify;\">The final decision diverges in several areas from the draft proposal CMS released for consultation. In finalizing the policy, CMS has removed the requirement for patients with a suspected white coat or masked hypertension to undergo behavioral interventions, such as diet and exercise modification, before receiving ABPM.<\/p>\n<p style=\"text-align: justify;\">Other revisions made by CMS also affect its policies on both white coat and masked hypertension. Notably, CMS dropped a requirement for ABPM devices to be certified and validated for use in the intended patient population by the dabl Educational Trust or a similar blood pressure monitoring quality control organization. CMS retained other requirements on the choice of ABPM devices.<\/p>\n<p style=\"text-align: justify;\">CMS also tweaked its definition of suspected masked hypertension. The final decision states patients are suspected to have the condition when they present with average office systolic blood pressure between 120 mm Hg and 129 mm Hg or diastolic blood pressure between 75 mm Hg and 79 mm Hg, plus at least two measurements of greater than 130\/80 mm Hg taken outside a clinical setting.<\/p>\n<p style=\"text-align: justify;\">The definition underwent minor changes in the finalization of the policy. The bigger change over the old text was put forward in the draft and retained in the final policy. Whereas CMS previously used 140\/90 mm Hg as the threshold for hypertension, it has now adopted the lower definition of 130\/80 mm Hg.<\/p>\n<p style=\"text-align: justify;\">Coupled with the coverage of ABPM for suspected masked hypertension, the threshold change could result in more patients being treated for hypertension. The medical groups that asked CMS to look at its ABPM policy argued changes to diagnostic practices were need in light of the cost of cardiovascular disease.<\/p>\n<h2 style=\"text-align: justify;\"><strong>Recognize the Types of Hypertension<\/strong><\/h2>\n<p style=\"text-align: justify;\">When <a href=\"https:\/\/medicalbillersandcoders.com\/Medical_Coding.aspx\">coding<\/a> these claims, make sure the code for ABPM, such as CPT 93784 Ambulatory blood pressure monitoring utilizing a system such as magnetic tape and\/or computer disk, for 24 hours or longer; including recording, scanning analysis, interpretation, and report is supported with an allowed diagnosis code; and that the diagnosis is in line with the new blood pressure threshold.<\/p>\n<p style=\"text-align: justify;\">\u2018White coat hypertension\u2019 is a condition in which a patient\u2019s blood pressure uncharacteristically rises in a clinical setting. Per the decision memo, suspected white coat hypertension is defined as An average office blood pressure of systolic blood pressure greater than 130 mm Hg but less than 160 mm Hg or diastolic blood pressure great than 80 mm Hg but less than 100 mm Hg on two separate clinic\/office visits with at least two separate measurements made at each visit and with at least two blood pressure measurements taken outside the office which are \u2018Masked hypertension\u2019 is when a patient\u2019s blood pressure is normal in a clinical setting but uncharacteristically rises at other times during the day.<\/p>\n<p style=\"text-align: justify;\">Per the decision memo, suspected masked hypertension is defined as: \u2018An average office blood pressure between 120 mm Hg and 129 mm Hg for systolic blood pressure or between 75 mm Hg and 79 mm Hg for diastolic blood pressure on two separate clinic\/office visits with at least two separate measurements made at each visit and with at least two blood pressure measurements taken outside the office which are \u2265 [less than or equal to] 130\/80 mm Hg\u2019. Both conditions are reported with <a href=\"https:\/\/www.medicalbillersandcoders.com\/ICD-10.aspx\">ICD-10<\/a>-CM code R03.0 Elevated blood pressure reading, without a diagnosis of hypertension.
